Please help with my Hepatitis lab results

Could you please review my health test and give suggestions please. I am but worried as last year my HBV DNA level was not detected but this year it is showing <20 IU/ml.

Hi @availlant , Thankyou so much for your response.

I am not taking antiviral therapy , I have detected Hepatatis B two years before and in last two year test results were HBV DNA target not detected but this year it is <20 IU/ml. I am bit worried should i go for more tests

Firstly, you have partial cure of HBV. Your HBV DNA is < 2000 IU/mL with normal ALT in the absence of therapy. Your long term prognosis is very good and you do not have to consider taking treatment.

Even when the HBV DNA test reports target not detected (TND), there is usually still a very small amount of HBV DNA present - its just beyond the ability of the test to detect.

Reverting from TND to < 20 IU/mL is a change which is not meaningful. Taking antiviral therapy will not give you a better result than this.

You should consider that there is always a very tiny amount of viral replication going on but that your body has almost complete control over it so that you will never develop liver disease.

You should continue to be followed every 6 months but please relax. You are ok.

Can the same thing happen to people who are on treatment but their Viral load not being detected?

Dear @Lilibet94,

Yes the HBV DNA test result can “wobble” between “target not detected” and <20 IU/mL. For some people, HBV DNA can persist at levels slightly above 20 IU/mL in the presence of antiviral therapy.

All of these results mean the same thing: stable control of infection with no danger of developing liver disease.
