New in this travel and very scared

Dear Thomas,
I just got back from my hepatologist and we sent tests for HVD to see if I have that too. Otherwise, he told me that it is either sadness or a small possibility due to the seroconversion of the e antigen.
If there is D along with B then things are much worse? It is common;

hello, I just got the results for D, antibodies completely negative and IgM negative. This would mean that I only have B and I am not infected by D, right? Must be run HDV rna test?

Hi @gvit,

If your anti-HDV antibodies are negative, then it is not likely at all that you have HDV infection. The HDV RNA test is usually a more expensive test and would probably not be worthwhile. If you continue to be anxious about it, there is always the possibility to do another HDV antibody test later.

Regarding how common it is, I think on average it is about 5-10% of people with HBV also have HDV.

Dear Thomas,
my doctor scared me because we have slightly elevated ALT and AST and he told me to see if it is related to D. Since it is negative everything is good for the time being, in fact I made a graph with my values while I am in treatment and I will quote it. Thank you from the bottom of my heart for your time.

Hello again I hope you are all doing better with your health. Today, after 2.5 years, I did elastography and my results are as follows: Em=7.88 kPa (7.65 in November 2020) E IQR/E median =22.4% Metavir Score F2=7.88 kPa. have I fibrosis? im in viread every day.

Dear @gvit,

These results would suggest that these is some liver scarring (or fibrosis). By limiting the virus replication with viread, you allow the liver to start repairing itself over time (this can take years).

Thomas

Dear Thomas
Thank you for your answer. This scares me that for 2.5 years I have been receiving my treatment, my load is below 50 IU/ml but the ALTs are not normalizing and I expected my liver to improve, but it is still a little worse. I have also done antigen e seroconversion and despite these my values are not getting better.

Hi @gvit,

This damage may have accumulated over decades, so it’s only fair to give your liver the time to repair itself.

Regarding the constantly high ALTs, has your doctor commented on this? As mentioned above, there might be other causes (e.g. fatty liver) that might be driving this.

Hi @Thomas and friends.
Since my hepatectomy which gave pathologists an opportunity to look directly at my liver and other samples., I have become pessimistic on radiogical and laboratory results we get. I have been reluctant to pose these but please allow me to endulge in it. For example before the procedure the ultrasound report indicated I had 6.8kPa fibrosis, yet the liver sample (biopsy) showed 4.0kPa.The size of the tumor on PET CT scan was 7cm but on surgery it was found to be 11×10cm.All tests ever done since I found out that I have hepb have given me thumbs up for my gallbladder. Funny enough during surgery it had to be removed because I suffered mild cholysticist(wrongly spelt)etc. Note I am referring to the same hospital and the same machines and the hospital is the best in Kenya and one of the best in Africa (Aga Khan University hospital)
Why these defferences

Dear @Kinoti,

You have been a great member of this community, so do not be afraid of posting anything that might be helpful to yourself or requests for support.

Re your liver, you mentioned in another thread that you had a 10x11cm section removed from your liver. Often surgeons will remove a larger part that what the tumour looks to be (a margin around the tumour) so that they are sure not to leave any piece of the tumour behind. This may be the difference.

Regarding your other questions, I’m not too sure, but may be answered by some @HealthExperts.

Hello everyone,
I hope you are all well. Today I went for an ultrasound of the upper and lower abdomen and in my liver he found a few microcystic elements up to 3.5 mm and ultrasound elements up to 3.5 mm that relate to small hemangiomas or calcifications, do you think this is something bad? My feet have been cut because 4 months ago he didn’t have these.

Hello @gvit

I understand your worries. I would be worried too.
It looks like you are doing the right thing, ultra scan, bloods,

The thing about having Hep B is, there’s no symptoms to Liver cancer.
So because you’re getting monitored, and they found something, not saying it’s cancer, they can now do something about it.
I have heard Patients having benign lumps, bumps, whatever. Including my sister.
One of our medical experts @ScienceExperts
Will help you better soon.
What did the doctor say?

Thank you very much @Caraline,
the doctor who did the ultrasound told me not to worry and the doctor who monitors me. I booked a CT scan tomorrow to be sure and on the Monday I will go to the capital to be seen by someone who is a specialist, I hope everything is fine and that our doctors here will also answer.

Hi @gvit,

I’m a scientist not a physician so I cannot comment in detail. However, I know that the liver can at times have benign lesions (I have a small cyst that the physicians are not worried about). I hope yours is benign too, and I wish you the very best as you work though this troubling time.

John.

Hello. I went to the doctor in the capital and he told me not to worry that it’s some simple cysts to continue with viread and my examinations. In fact, he told me that from September he will be able to put me on the GSK drug, do you want me to participate? has anyone received it? it has a result; does it have side effects?

Which GSK drug are you referring to?

John

Hi @gvit
That is good news.
I don’t know what GSK is. Can you explain please?
Glad to hear from you again.

its tha Bepirovirsen if i understund right in Phase 3

Dear @gvit,

In the previous reports from trials of Bepirovirsen , the common side-effects have been generally mild (“injection-site reactions, pyrexia, fatigue, and increased ALT levels were reported more frequently with bepirovirsen than with placebo”). There is a 10-15% reported rate of more serious side-effects if you are also taking ETV/TDF.

There appears to be about a 10% chance of HBsAg loss with the tested treatment regimens (this may be different in the new trial)

Trial results here: https://www.nejm.org/doi/full/10.1056/NEJMoa2210027

Hope this helps,
Thomas
