Low numbers on HbsAg

Hi, What would be considered as LOW for HbsAg? I plan to go for Bepiroversin jabs as I read in the forum that it could work well with a low HbsAg. Thank you.

Welcome to the community. I will say anything less than 2000 or about max 5000. It can increase the chances for functional cure with some of the current antivirals especially below 2000. Best, Bansah1.

That sounds good! Currently my HbsAg levels stands at 854 IU/mL, after 11 long years on Tenofovir (from a starting high of 2550 IU/mL) !! The HbsAg numbers you mentioned are in IU or copies? Mine is the mutant form though, HbeAg negative. Would this mutant form change the outcome? Thank you.

It’s always in measured in IU/mL. I believe the mutation might make it difficult, but from your results it seems possible if your number have dropped to that level. Only 1-5% of patients attain functional cure with the current antivirals we have. So a very small sample or percentage. Best, Bansah1

Thanks for the feedback. I will wait another couple of years. My numbers may come down to about 500 by then and at the same time we’ll have the results from the current Phase 3 trials on Bepiroversin. I may be able to make a better decision at that point in time.

I went for this treatment during one of their trails. The first 3 months was full dose and the last 3 months was half. During the treatment the Hbs quantity went down from 7000 iu to zero. But after stopping the treatment it went back up and it remaind after 1 year post treatment to around 2000 ui. Therefore in my view the gain is huge. Also 2 years post treatment HbeAg became negative and hopefully the anti hbe will appear in the future.

Because it was a trial they were doing last of blood test to be sure there is not major adverse event.

The impact on me was: low pallets count, small impact on hemoglobin and some fatigue + ast and alt in the 100 range. Otherwise a really easy to tolerate treatment.

Also kidneys need to be monitored with attentio.

Looks like the jabs did their part well in your case! One question if you may want to share: Were you on NUC’s before the Bepiroversin jabs and if so what were your HbsAg levels before starting NUC’s?

I started Viread in 2012 and interferon in the same time. At the start point in 2012 hbsq was 70 000 iu. After 1 year of interferon and Viread it went to 50 000 iu. Then year by year it dropped. From 2019 I moved to Vemlidy. And while on bepirovirsen I was also on Vemlidy. And now I’m continuing as well Vemlidy.

Starting point of HbsAg at 70,000 and currently sitting at 2000 (12 years later), is a huge gain - a 97% DROP - an amazing turnaround! I started Tenofovir at around the same time as you (2013) and the drop so far is sitting at only 67%, despite a start off point of a low HbsAg at 2550. Mine is the wild mutant type though! I have not been on any other treatment as yet except for Tenofovir all these years - no side effects whatsoever!

Are you taking any special diet to reduce your hbsag level… you smoke or consumed alcohol in these years … are you doing any workout … or anything special to reduce it… i was diagnosed in mar 2024 with the hbsag level of 2800… now i am on TAF 25mg … what precautions did you take ???

Hello, I quit smoking on 2017 and drinking occasionally 2 years back. I think is critical to not drink and smoke.

Diet: nothing special. Exercise: everyday walks of 2-5 km.

What I have also done, is to take shots of imunoglobuline of hepatitis antibodies. But this you can’t due it without medical supervision as is risky and is no evident proof that works.

Agree its difficult to quit alcohol and smoke , so now you are consuming alcohol ocassionally… ?? Your hbsag was 70000 iu/ml or 7000 iu/ml which has been reduced to 2000 iu/ml…??

In 2012: was 70 k. Over the years till around 2020 it was reduced to 7k, then started the GSK trial, it was reduced to zero and bounced back to around 2k.

Zero alcohol and smoking. Because if you don’t do that, then you alter negatively your potential to clear the infection in a future future.

You have been to GSK trials…. Are we expecting functional cure soon ???
