HBV with invasive procedure during pregnancy

Hello everyone, I am new here and expert opinion is needed please.

I only discovered that I am HBV+ when I was pregnant 3 years ago but prior to that I and my husband are both AS by genotype and we already carried out CVS on the pregnancy when I was 12weeks pregnant.

The results came out good as my baby is AS too, when I found out about my HBV status the doctor said I should carry out some tests which showed that I couldn’t really transfer it at that time so I wasn’t plane on any medication, my baby took the Hepatitis B immune globulin at birth and the first shot of vaccine 4days after. I haven’t tested him for it yet because I’m a bit scared of the outcome.

My major issues now is that we are planning to have another baby that will also go through the same CVS procedure and the doctor has ordered some tests which the results are below.

Please I need interpretation for the results and my question is that can the cvs invasive procedure pose risk of contraction from mother to child? By the way my husband is negative two years ago and he’s still negative, he’ll also get vaccinated soon.

@Bansah1 @ThomasTu and other people please help!

Hi @Flo,
Welcome to the community and thanks for sharing your concern. I bet it might be really stressful hearing you will have to go through this process again. I really empathize with you and your concerns are valid. We will all worry if placed in your shoes. Your lab results indicate that you have hepatitis B with a low viral load. There is no evidence of viral replication going on (HBeAb positive) which is good. Your viral load is very low, that the rate of transmission is going to be less likely (or very low).

About the CVS procedure, I am not familiar with it but from a little reading on this it sounds like they are taking a sample from the placenta for further testing. Given your low viral load, I don’t think the procedure increases the chances for transmission. Mother to child transmission is less likely here I would think given your low viral load of 317. The lower the viral load the less likely it is and the higher the viral load the more likely it becomes. As long as your viral load remains low and there is no replication, I believe it should be fine. I am glad to hear your husband and first child are both vaccinated. @Thomas can comment on this as well.

I hope this is helpful. Bansah1.

Thank you so much for taking time to reply, my doctor also said she’s not going to start any treatment for me for now.

Sure, because of how low your viral load is I am not sure treatment is right here. If you start treatment, I am not sure you can stop it without increasing HBV flare-ups. I thought about this and I kind of agree with your doctor on this. Try and talk to your doctor to take extra precautions during the procedure so they can protect and prevent any contaminations or errors. We are all here for you. Keep us posted. Bansah1

Hi @Flo,

Thanks for the great question and sorry to hear about your worries. Maybe this is something that one of our @HealthExperts can answer better.

In the meantime, from my reading, there aren’t huge numbers of studies about this, but those that have been published show that low HBV DNA levels and HBeAg-negativity (like in your case) have very low levels of risk involved in transmission:

Hope this helps,

Welcome @Flo
Thanks for sharing. Considering your anxiety about transmission, I would make your husband’s vaccination a priority.
We all go through the worry of transmission to others, and I’m surprised that you have not had your husband vaccinated.
Maybe I have missed read your comments. Forgive me if I have.

All the best for your coming Child.

Thank you Thomas, I read through the link that you sent, although it’s a bit helpful, I didn’t find any of the cases that exactly match my own situation. I’m glad you replied, thank you.
