HBV DNA Fluctuations and ALT Rise — Seeking Guidance

Hello everyone,
I’m 37 years old and was diagnosed with chronic hepatitis B in 2015 during a routine test. I’ve never used drugs and I don’t drink alcohol.

At the time of diagnosis, my doctor only suggested checking liver enzymes, which were always normal. In 2020, I decided to check HBV DNA myself — it was 70 IU/mL, with normal liver enzymes.

Since then, I mostly monitored liver enzymes, which stayed fine. In June 2025, I began taking my condition more seriously and found a hepatologist. He checked HBV DNA: 80 IU/mL, liver enzymes normal. In September 2025, I had an elastography, which was totally normal. In October 2025, my annual exam again showed normal liver enzymes.

Now, on December 1, 2025, my HBV DNA is 103 IU/mL. Liver enzymes are normal except ALT 37 (reference value 32). I’m waiting for my doctor’s call, but I feel panicked because my enzymes have never been raised before.

My questions are:

  • Is this kind of HBV DNA fluctuation (70 → 80 → 103) considered normal, or does it mean reactivation?
  • Could the mild ALT rise be connected to the HBV DNA changes?
  • How do others interpret these small shifts in numbers, and what helped you feel confident about monitoring?

I’d be grateful for any insights or experiences. I’m learning to balance proactive care with calmness, and hearing from others would mean a lot.

Your HBV DNA is very low, and the fluctuations you described are not considered significant. Generally, the guidelines for treatment are if you have HBV DNA above 2000 IU/mL, along with ALT >2x upper limit of normal. I’m not sure if the ALT rise is related to the HBV DNA levels, but it’s normal for both to fluctuate a little bit. I’d recommend continuing to monitor every 6 months.

I don’t know if you maybe increased your activities / sports?

Strenuous exercise like weightlifting and strength training can temporarily increase liver enzyme tests (AST, ALT, AP and GGT). It is recommended to abstain from exercising for 7 days prior to having blood tests taken. I know it doesn’t explain the fluctuation in HBV numbers but I wanted to share this info about the liver enzymes.

Thank you for your reply. The whole week before my enzyme test, I increased my exercise—mostly running, not weightlifting. I’m not sure if this could raise only ALT, since AST and the other values are still within the normal range.
