F0-F3 plz help!

I recently get fibroscan with 8.9kpa
I had fatty liver last yr … a year ago my fibro was 5.7kpa with 269 cap score…

My hep b is under control - undetectable…

What else can elevate my fibrosis? I have been drinking lately is that the case? Or maybe my fatty liver getting more advanced? Recent fibroscan did not give cap score. I am too worried since 8.9kp is too close to F3… is this reversible??
plz let me know what your take away is…

Thanks

An increase in FibroScan from 5.7 kPa to 8.9 kPa could be due to recent alcohol consumption or progression of fatty liver. Reducing alcohol, improving diet, and exercising can help.

Consult your doctor for a comprehensive plan and continue monitoring your liver health.

I once had cirrhosis of the liver . With exercise and diet the liver reverse back normal and the fibroscan scored 0-1

What type of exercise u used to do… @Ccheng8346 ?

Thanks

Hi @ Nass
Exercise with control your diet eg: Mediterranean diet with healthy lifestyle can reverse your fatty liver especially with targeted weight loss about 7-10% .
The type of exercises recommended as the link below. Physical activity in metabolic syndrome - PMC. I hope this may help and benefit to your liver health.

I had a similar fibroscan result. I then took n-acetyl-cysteine for a week and felt a lot better, then got another fibroscan, and it showed no liver fibrosis. That led me to think that perhaps it was measuring congestion/inflammation more than fibrosis, I guess both of which can increase liver stiffness in the readings. I took about 1-3 grams a day and it made me feel better overnight. This has never been tested in a clinical trial so it’s just my anecdotal experience

There are also other liver diseases to consider other than fatty liver and it is worth excluding other potential causes (e.g. HDV or HCV) that could be managed or cured.

Thanks @ThomasTu
I have done all those exams… none…
A yr ago, my fibrscan was 5.7 with 269 cap score…

This time i went to another place that use fibroscan with no cap score capability…. I am very doubtful about this results… it says… 8.9……all my liver enzymes are normal… my AFP was high 16 a yr ago, now it’s 7… almost normal… I have also did HCC panel test all fine….

I am going back to previous fibrscan doctor to see what he says…

Thanks

Coincidentally I also have raised afp, ranging between 15 and 45 for the last decade. I was reading some research from the last few years saying that the more up to date thinking when it’s at these levels is that it’s a marker of regeneration (and it showed an inverse correlation with fibrosis) and a positive sign in general, which is a more optimistic view than the previous thinking of it being more associated with HCC. But now I can’t find it again

Did you ever try NAC?

Hi @bob
No I have not tried NAC… I will probably ask my doctor about it….

Plz try to find out that Study regarding AFP… its good topic to discuss with experts in the forum…

My AFP was 16 a yr ago… then gradually decreased to 14, 12 and now 7.3…

The normal AFP should be 4.5 and less for Hep B patients…but for normal people it can be up to 10…

Thnx
Nas

@bob
I met my doctor this morning and he kind of agreed that the AFP increase could be a marker of regeneration!!

Have u done another fibroscan? I had a fibroscan done and it says 17, all my enzymes are normal from 2years now and ultrasound always normal, this is the first fibroscan i would do, im a bit confused and sad now, is this a cirrhosis level? HBV dna is just 38, they keep saying I don’t need meds is this reversible? I’m scared right now…

Hi @anonymous83

Is fibroscan 17? That seems high … but I don’t trust first scan. Plz get with your doctor ASAP and figure out …

Hopefully our medical group can comment on this…
@ThomasTu

Seem so high, so I feel maybe because I ate before the scan, maybe some technical issues, cuz I’m not really certain fibroscan can be that high without any complications or show anything like raised alt and ast in the blood test and ultrasound… got blood work done so I’m waiting for the result, I’m seeing my doctor again by may 1st, I don’t drink alcohol nor eat junks or fried stuffs and I don’t expect just 38 viral dna get to this very high fibrosis. The liver nurse said it’s F2 but that seems confusing again, how can fibroscan 17 be f2? That’s f3 or early f4 in my opinion, they don’t seem to be concerned at all and were still insisting I don’t need meds which baffles me cuz I feel even if the guidelines is that treatment shouldn’t be given to someone with less than 2,000 viral dna, if there’s damage they should be thinking of treatment or something…

Hi anonymous 83
Regarding Fibroscan test , there are many factors that affect the results. False positive results may be found in no fasting condition before procedures, elevated AST/ALT> 100 IU/ml , liver congestion or IQR of standard machine. However, we can use other non-invasive test eg. APRI and FIB4 scores by online calculator to help in assessment of fibrotic stage of liver.
In your case with fibroscan results show 17 kPa. If we use rule of four ((5, 7 ,9 , 12 kPa) for simple assessment which equal to Metavir stage F1 , F2 , F3 , F4 respectively in chronic hepatitis patients.
Fibroscan (LSM) results with 17 kPa is consistent with cirrhosis. APRI score > 1.5 and FIB -4 > 3.25 have high positive predictive value (PPV) and can rule in or confirm liver cirrhosis. If you calculate APRI and FIB-4 and the results are discordance with fibroscan result,
I don’t think you’ll have cirrhosis of liver. You should consult with your doctor for definite evaluation of fibrotic staging of your liver. I hope this may help you.

I’m just trying to stay positive and hopeful, blood sample has been gotten and result is not ready yet, I’m having another appointment on Thursday…
I ate before the scan, that might be a reason for that high score, I just hope it is, I have always been having scan and blood test for all liver enzymes and there’s never been a time where I have anything outside of normal, everything has always been good, the result ultrasound was good, viral dna is 38, this is what baffles me, Maybe im to emotional about the whole thing cuz I have not had a proper sleep since that day, I don’t drink alcohol and I don’t eat junks or fried stuffs, so this is just strange out of the blue, the liver doctor was saying F2, I don’t know if it’s possible to tell me it’s f2 if it’s truly f3/f4, why did she say it’s f2 cuz I categorically asked what level/stage of fibrosis is that score… I’m going in on Thursday and I’ll have to ask series of questions, she kept saying I still don’t need meds and In some of the research I have done, I think they still treat people if they have F2/f3 so why did she say I did not need meds even at this level? APRI and FIB-4 online calculator says 0.4 and 1.32 respectively means both are normal and in f0-1 level… but a new blood test is done to see if there’s reactivation from last 5months,even at that with just 38dna I don’t think there’s should be any high score like that… finger crossed

How long does that take u? And did u take any meds? Any diet plan?
