Appointment clarification

Hi all,

I was diagnosed with last year in September of 2024 and was told I have a chronic infection and since then I have been going to my appointments and getting my scans. Life has not changed much and I have learned to live with this.

the only thing is I get anxiety when I get here and today I just had an appointment.

I need some help understanding my results as the doctor was new and did not give me much answers.

what is concerning to me is that the doctor told me my HBV DNA is gone down from 1300 iu/ml to 400 iu/ml and I’m not on treatment.

My last fibroscan showed that I was at 3.1kpa and my lfts and alts remain normal and within range. My platelets count though is at 133k which they seem a little concerned about.

also they said my ultra result showed that my liver had irregular surface or something and I was concerned and wanted to start treatment because surely this means fibrosis or cirrhosis or something?

You are not alone in feeling anxious when you are about to attend your appointments. Many of us feel the same way. I think it is the fear of the unknown and us wondering what the results could be, saying or showing. We become anxious in reaction to not knowing the results. This is a normal human reaction, I will say. Try practicing deep breathing. Take a deep breath through your nose, hold it for a few seconds, then breathe out completely through your mouth. It should help calm you down a bit.

Your viral load decrease is good; it means your immune system is controlling the virus. This is a good sign since you are not on treatment. It is hard to say much about your ultrasound without seeing the report. Having an irregular surface can be anything from cysts, fat, fibrosis, etc. Are you able to get and share the report of the ultrasound?

I don’t know why your platelet count is on the lower end. You should keep your eye on it.

Thanks @Bansah1 , I don’t habe access to my reports the doctor does not give them to me to see or email them over. I am trying to to get them to do this.

it was a new doctor who at first did not give me the correct information regarding my HBV DNA and had to go ask another doctor to confirm. I missed one ultrasound before so she was looking at one from January, I am confused because when I went in back in April of this year the other doctor said my ultrasound looked good(this is the same ultrasound this doctor looked at) so now I’m confused.

Your virus should be undetectable . Start taking viread or Vemlidy. Monitor your HbsAg ideally <100. Monitor your ALT <25 means your liver cells are not inflamed . You can lower your ALT if you stay on elliptical ( exercise 30 min daily ) . Try to find what genotype your virus is . Genotype C is more aggressive than B .

I feel what you go through, you are not alone. Positivity is the key. Our immune system tends to act or behave proportional to how we feel mentally. Dont stress youself. You seem to be controlling the infection well as per now but you need to monitor your liver as stipulated by the hepatologist.

Thanks @TomA yeah I think I’ve calmed down now and not too fussed as if there was any major concerns they would have ordered more tests and the doctor did seem quite in experienced.

I was just concerned as I don’t drink or smoke and I’m generally healthy so was concerned about what was going on but I’m happy and privileged that I live in a country where this is free and I have access to all the best technology to monitor these changes so I will be positive on that and hope for the best :slight_smile:

Hi all,

I’m a person living with chronic hepatitis B (HBeAg-negative, anti-HBe-positive). I’m seeking guidance because my recent viral load has increased and I’m feeling anxious about what it means. Here are the details:

  • My viral load history:

    • October 2024: 1,300 IU/mL

    • April 2025: 490 IU/mL

    • September 2025: 2,100 IU/mL( the doctor noted that this is a significant increase on my notes)

  • My liver function tests at the last check (October 2025):

    • ALT: 19 U/L

    • AST: 22 U/L

    • Bilirubin: 17 µmol/L

    • ALP: 81 U/L

    • GGT: 25 U/L

  • Liver ultrasound: normal, no focal lesions, spleen slightly enlarged at 15 cm.

  • Relevant context:

    • I experienced stress and depression during the summer.

    • My diet was higher in processed and fatty foods, and my exercise routine was reduced compared to earlier in the year.

    • I have been highly stressed and eating more sugar less sleep before

    • I also started Mounjaro (tirzepatide) recently.

My concern is whether this viral load increase is something to panic about, or if it is likely a temporary fluctuation given my healthy liver tests and ultrasound.

I would appreciate any guidance on:

  • How worried I should be about this rise.

  • Whether this trend warrants earlier blood tests or starting antiviral medication.

  • Any advice for supporting my liver and immune system while waiting for my next scheduled appointment.

There is an increase in your viral load. While this is not very high, I will encourage you to have a conversation with your doctor, especially the fact that you are taking other medications. While this increase is noticeable, it is something to keep an eye on, especially to see if it decreases again or continues to rise.

The same old stuff, moderation is crucial in everything. Eat healthy, get good sleep, avoid drinking and smoking, exercise, decrease and manage your stress levels, etc. I hope this helps, Bansah1
